Care Coordinator, New Hampshire.
Care Coordinators are mental health professionals who provide comprehensive, local support to Service Members and families in the context of a supportive professional relationship. Care Coordinators meet with Service Members and families in their homes and in other local settings at times that are convenient for the Service Member and/or family. Care Coordinators are trained in counseling and service coordination, and meet the continuing education requirements.
